Carlo Fornaini è nato a Treviso il 28/12/1954.
Si è laureato in Medicina e Chirurgia nel 1980 presso l’Università di Milano ed ha svolto il Tirocinio pratico ospedaliero nell’80-81 presso la Divisione Odontoiatrica dell’Ospedale di Parma.
Ha frequentato numerosi corsi di aggiornamento in Italia e all’estero.
Nel novembre 1995 ha conseguito il diploma al Corso di Perfezionamento in Ortodonzia Intercettiva presso l’Università di Pavia.
Nel luglio 2000 ha conseguito il diploma al Corso di Perfezionamento in Patologia dell’ATM presso l’Università di Milano.
Nell’ottobre 2002 ha conseguito il diploma al Corso di Perfezionamento in Odontoiatria laser presso l’Università di Firenze discutendo una tesi su “Utilizzo dell’Er:YAG nelle fratture coronali esposte”.
Nel 2005 ha conseguito il DU in "Oral Laser Applications" presso l'Università di Nice Sophie Antipolis (Francia).
E’ Attaché Universitaire de Recherche presso l’Università di Nice Sophie Antipolis (Francia).
È Professore a Contratto e  Docente di “Applicazioni del Laser in Odonotstomatologia Clinica”  presso il Corso di Laurea in Odontoiatria e Protesi Dentaria presso l’Università di Parma.
È Visiting Professor presso l’Università di Liegi (Belgio).
È Coordinatore di EMDOLA (European Master Degree in Oral Laser Applications) presso le Università di Parma e Nizza e Docente di EMDOLA presso le Università di Parma, Nizza, Liegi, Roma La Sapienza e Barcellona.
È relatore a congressi e corsi in Italia e all’estero ed autore di pubblicazioni in riviste scientifiche italiane ed internazionali.
Ha collaborato con il prof. Paolo Vescovi alla redazione del volume "Osteonecrosi dei mascellari e bifosfonati" e con il prof. JeanPaul Rocca alla redazione del volume "Les lasers en odontologie".
È Fellow dell’ Academy of Dentistry International.
È socio ANDI, SIDO, SILO, IALMS e WALT.
È tesoriere dell’European Division di WFLD (World Federation for Laser in Dentistry).
È Consulente Tecnico presso il Tribunale di Piacenza.
Esercita la libera professione odontoiatrica a Fiorenzuola d'Arda (Piacenza).
